Many neurosurgical lesions are simple and can be figured out quickly with a few cases (e.g., spinal dural AV fistulas). Others are complex and common, and we gr...adually figure them out (e.g., brain AVMs). But some are both complex and rare, and it takes shared insights to figure them out collectively (e.g., spinal cord AVMs). Late-week surgery and discharge to specialty care associated with higher costs and longer lengths of stay after elective lumbar laminectomy by Sebastian Salas-...Vega, Vikram B. Chakravarthy, Robert D. Winkelman, Matthew M. Grabowski, Ghaith Habboub, Jason W. Savage, Michael P. Steinmetz, and Thomas E. Mroz New research by a team from the Cleveland Clinic and the London School of Economics and Political Science (LSE) has determined that surgeries performed late in the workweek, and those culminating in discharge to a specialty care facility, are associated with higher costs and unnecessarily longer stays in the hospital following a common elective spine surgery. See full details in this freely accessible article: https://doi.org/10.3171/2020.11.SPINE201403 Composite figure based on findings from the article by Salas-Vega et al., which show that elective lumbar laminectomies occurring later in the workweek are associated with longer hospital stays (in days).

In this operative video, Dr. Cohen demonstrates techniques for resection of a giant falcotentorial meningioma. After removal of the tumor, the section of the f...alx invaded by the tumor may be excised using a curved knife to cut the falx toward the superior sagittal sinus. The inferior sagittal sinus may be sacrificed if the superior sagittal sinus is patent. Extension of the tumor to the contralateral side of the falx requires transection of the falx and creation of an operative window to reach the tumor. Alternatively, the craniotomy may be extended to the other side of the sinus and the tumor exposed through a contralateral paramedian dural opening. See the pearls here: https://zcu.io/B76i
